It comes with the original box, instructions, and charging dock, but no USB cable. It is from a non-smoking home/environment.
This phone has had one previous owner. It is not unlocked, but I'm sure it can be. It is in perfect working condition, and the screen is in really good shape. It's an awesome phone to have, with the ipod built in and a great variety of applications (tons of them FREE) to download.
